  • Which conditions qualify me for a medical card?

    You may be eligible for a medical card if you’ve been diagnosed with one or more of the following severe, debilitating, or life-threatening conditions:

    • Cancer
    • HIV infection or AIDS
    • Amyotrophic lateral sclerosis (ALS)
    • Parkinson’s disease
    • Multiple sclerosis
    • Spinal cord injury with spasticity
    • Epilepsy
    • Inflammatory bowel disease
    • Neuropathy
    • Huntington’s disease
    • Post-traumatic stress disorder (PTSD)
    • Chronic pain, defined as pain that degrades health and functional capability
  • How long does it take to get my medical card?

    Once your doctor has approved you for a medical card, you will receive a pdf copy of your certification via email within 24-48 hours. With this certification, you will complete your state registration. After your registration is approved you will be issued a Temporary Registry Identification Card, which you can use to purchase your medicine until you receive your registry identification card in the mail.

  • Which documents will I need for my evaluation?

    You’ll need to provide your medical records, proof of identity, and proof of residence. Your New York driver’s license or ID can prove both your residence and identity (if it’s up to date). If you don’t have a driver’s license or ID, a current utility bill or mail from the government addressed to your current address will be enough.

  • How much does it cost to be evaluated by a physician?

    The cost of your evaluation will depend on the physician, but will generally range anywhere from $99-$299.
